Please fill out our patient paperwork here. You can also find our paperwork under the New Patients tab. Please provide your dental insurance information at this time so we can verify your benefits. It is also recommended that you verify these benefits with your insurance carrier or your HR department to ensure accuracy.
During your 60 minute complimentary visit you can expect the following:
- Digital photos and optional digital 3D scan with the itero scanner
- Panoramic x-ray (if your dentist does not have a current one)
- Discussion of your concerns and goals with a treatment coordinator
- Exam and treatment recommendations with Dr. Garfinkle
- Presentation of treatment fees and next appointments
If after your exam you are ready to start your treatment, we are happy to collect any remaining diagnostic records and financial paperwork in order to expedite the process! We offer in-house interest free payment plans or a pay in full discount.
Many children who still have primary teeth are not ready for treatment and will be put into an observation phase where Dr. Garfinkle can monitor growth and the eruption of permanent teeth. These yearly visits are complimentary.

What Happens After The Initial Consultation?
You’re one step closer to achieving the smile you’ve always wanted! After your consultation, we will schedule an appointment to complete any remaining diagnostic records and a subsequent visit to start your orthodontic treatment. These appointments generally take 30-60 minutes. During this time our team will be available to make sure that you are as comfortable as possible.
Visiting the orthodontist every six to twelve weeks is an important part of your orthodontic treatment. Our practice will work with you to make sure that your appointments are as convenient as possible.
We understand that “life happens”! If you’re running late or need to reschedule, please let us know and we will do everything we can to accommodate you.
Visiting The Dentist During Orthodontic Treatment
Please continue to visit your family dentist every six months, even during your orthodontic treatment, for teeth cleanings and dental checkups. If extra dental care is needed, we will be happy to coordinate with your family dentist to make sure that you are receiving the best care possible.
